Pareto efficiency is (essentially) the idea that if you can't make someone better off without making someone worse off, then pareto efficiency is achieved. The comic is referring to the fact that he feels certain sexual acts could be reciprocated simultaneously and make both parties better off, so doing the act one at a time is not efficient, and therefore not an option for Vilfredo.
