I've been reading this comic for years and you just made me realise there is a title text. That will need a couple of days without sleep to read them all.

He didn't start adding the alt-text until a couple years ago. He's also been going through all the comics starting at the beginning to add voteys. Yummy yummy ad revenue.

I like how in this comic, the Fireman is crying. If he had just said "Ha ha! Made you say underwear!" then it would have been another comic about bad bedside manners, but because he is crying it suggests that the Fireman is retreating into humor to mask the pain.
